The forms listed on this page are the official, most current revisions, as incorporated into the Commission’s regulations.
SPECIAL NOTICE: The Bureau recently revised the amounts of the various deposits for its background investigations and processing costs. Those revisions were included in amendments to Title 11, California Code of Regulations, Section 2037, which were approved by the Office of Administrative Law and filed with the Secretary of State on March 16, 2011. The revised amounts become effective on April 15, 2011. Consequently, any background deposit amounts or other Bureau fees specified in any of the following forms may be incorrect. * These forms are exempt from the rulemaking provisions of the Administrative Procedure Act (Gov. Code, Title 2, Div. 3, Pt. 1, Ch. 3.5, § 11340 et seq.) and, therefore, are not incorporated into the Commission’s regulations. The forms listed are the official, most current revision.
